From Route 146 South: Take the Rt 16/Uxbridge/Douglas exit. Take a left at the bottom of the ramp onto Rt 16. Travel 3/4 mile and take a left onto Cross Road immediately after the Uxbridge Police Station. Take a left onto Hazel Street and travel approximately 1.5 miles and the hospital is on the right.

From Route 146 North: Take the Rt 16/Uxbridge/Douglas exit. Take a right at the bottom of the ramp onto Rt 16. Travel 3/4 mile and take a left onto Cross Road, immediately after the Uxbridge Police Station. Take a left onto Hazel Street and travel approximately 1.5 miles and the hospital is on the right.

From Route 122 North: Travel through the center of Uxbridge and continue on Rt 122 toward Northbridge. Pass the Hannaford Shopping Plaza on the left. At next lighted intersection, take a left onto West Hartford Avenue (at this corner, the new CVS Pharmacy is on the right and a Getty Gas Station is on the left). Follow West Hartford Avenue for approx. 2.5 miles up a very large hill and pass Bangma's Dairy Bar. The next left after Bangma's is Hazel Street, take this left and the hospital is on the left about 1/2 mile down.

From Route 122 South: Travel on Rt 122 towards Uxbridge, you will pass Brian's Restaurant on the right. At the first lighted intersection, take a right onto West Hartford Avenue (at this corner, the new CVS Pharmacy is on the right and a Getty Gas Station is on the left). Follow West Hartford Avenue for approx. 2.5 miles up a very large hill and pass Bangma's Dairy Bar. The next left after Bangma's is Hazel Street, take this left and the hospital is on the left about 1/2 mile down.
